A NATURAL PATH TO HEALTH PRIVACY, COMMUNICATION, and HIPAA POLICY

A Natural Path to Health, LCC and its associated health professionals collect, use, and maintain personal and medical information for treatment, payment, and healthcare operations under HIPAA privacy regulations. 

PROTECTED HEALTH INFORMATION (PHI) UNDER HIPAA

Your protected health information is confidential and will only be disclosed to third parties with written permission, except as permitted or required by law, including:

  • For treatment, payment, and healthcare operations
  • When required by law or court order
  • For public health activities
  • To prevent serious threats to health or safety
  • For healthcare oversight activities

SMS/TEXT MESSAGE COMMUNICATIONS

Patients are required to consent to receive appointment reminders and other healthcare-related communications via SMS text messaging to their designated phone number.

  • Message and data rates may apply.
  • Message frequency depends on healthcare needs and appointment schedule.
  • Patients can opt out of SMS/Text messages at any time by declining consent requests or contacting our office.
  • SMS communications may not be encrypted and could potentially be accessed by unauthorized parties.
  • The patient assumes this risk by consenting to text message communications.

Upon SMS consent, the patient agrees to receive SMS text messages communicating appointment reminders and general support from A Natural Path to Health, LLC, and Dr. Michael Conway, ND. The message frequency may vary. Message and data rates may apply. You can reply STOP to opt out of further messaging, or reply Help for help, or call our office at (360) 695-7699 for assistance.

FCC Compliance: No mobile or messaging consent information will be shared with third parties/ affiliates for marketing/ promotional purposes. Text messaging originator opt-in data and consent will not be shared with any third parties.

INFORMATION COLLECTION AND USE

We collect only the personal and medical information necessary to provide healthcare services, including:

  • Personal identifiers (name, address, phone, email, insurance information)
  • Medical history and current health information
  • Treatment and billing records

This information is used solely for:

  • Providing medical care and treatment
  • Processing insurance claims and billing
  • Appointment scheduling and reminders
  • Coordinating care with other healthcare providers

DATA SECURITY AND RETENTION

We maintain physical, electronic, and procedural safeguards to protect patient health information in compliance with HIPAA security requirements. We retain medical records as required by law and professional standards, typically for 10 years after the last visit.

RIGHTS Under HIPAA are:

  • Access and receive copies of medical records
  • Request amendments to health information
  • Request restrictions on how information is used or disclosed
  • Request alternative methods of communication
  • File a complaint if it is believed that privacy rights have been violated
